Accounting Student Name Badge Form
If you are a student and want to purchase a Kwantlen name badge you can:
1. Download the appropriate form
2. Complete the form and fax to the printer (the fax number is on the form)
*Note, name badges will be delivered to the Surrey printshop around the 10th of every month
3. Go to the printshop after the 10th of the month.

8:00am - 4:00pm
Lunch Break 12:00pm - 1:00pm
Surrey Campus, Birch Building, Main Floor
4. Bring $8 cash, exact change only
5. The printshop will give you your name badge
